Care Management
The Care Manager at Elder Care Resources provides a variety of services and oversight to assist you and/or your loved one to handle personal care, health care, administrative tasks, medical appointments, and social/safety/daily living needs of older adults.
Paperwork management and organization
Working with insurance companies
Medical and family liaison
Status reports to interested parties via text message or email
Monitor medications
Advocate in medical and legal appointments
Assistance in choosing a long-term care facility, placement and admission
Wellness visits
Coordination of other service providers
Consultation and supportive counseling
Elder Care Resources offers in-home caregivers to provide assistance so that our clients can age in place, including:
Companionship
Respite care for family
Light housekeeping
Laundry
Light meal preparation
Medication reminders
Showering and dressing assistance
In-Home Care Giving
